• Limited period of support. One beneficiary requires – six visits for just six weeks
  • Beneficiaries often live alone and have been up to 100 years old
  • Volunteers regularly support 1, 2 or 3 beneficiaries per week taking approximately 1 hour each visit

Pivot Home from Hospital is our project based in Pembrokeshire. It supports people, mainly older adults, who have recently been discharged from hospital and would benefit from a bit of extra help at home.

Each person is matched with a volunteer who can provide one or more of the following:

  • Face-to-face or telephone befriending

  • Light shopping or picking up prescriptions

  • Dog walking or light gardening

  • Even small tasks like running the vacuum round the living room

It might not sound like much, but this friendly, practical help can make a big difference to someone’s recovery and wellbeing.

We are not just here for a chat. During the six weeks of support, we also signpost or refer people to other services that might help them long term.

We can put people in touch with:

  • Local home adaptation services
  • Financial assessment support
  • Community Connector programmes
  • Mental health support such as Mind
  • Local groups such as the 50+ Forum

All referrals must come from a professional. This could be a hospital team member, a GP, physiotherapist, or occupational therapist.

We currently only accept referrals from the Coordination Centre at Porth Procelli Pembrokeshire.
