Pivot Home from Hospital is part of our commitment to reduce social isolation, improve wellbeing and empower people to lead change in their communities.


As a Community Volunteer with Pivot Home from Hospital, you will provide light-touch, practical support to people, mainly older adults, as they transition from hospital back into their homes. This could include friendly visits, help with small tasks, or simply offering reassurance during a time when confidence may be low.

Each person is matched with a local volunteer for up to six weeks, helping them feel safe, settled and connected. Even a small amount of support can make a significant difference to someone’s recovery and independence, and may help prevent a return to hospital.

You will be fully supported by our engagement managers as part of a welcoming and committed volunteer team. By giving your time, you will play a vital role in strengthening communities, reducing social isolation and helping people live well at home.

  • telephone or face-to-face befriending
  • picking up shopping or prescriptions
  • dog walking or light gardening
  • help with small tasks like vacuuming
  • support with our ‘keep warm, keep well’ scheme in autumn and winter
  • being a travel companion to appointments or social activities

The role description is intended to include a broad range of responsibilities and requirements of the volunteering post. As our services are continually changing, there will be regular opportunities for you to change the nature of your volunteering and / or support other services in our area to ensure that you continually gain more skills alongside positive experiences while you are volunteering with us.

Volunteers are the cornerstone of our work with the residents from our local communities and we are grateful that you are offering your time to help us to make a difference.

The PIVOT (Home from Hospital) project supports patients in and around Pembrokeshire who have been brought home from A&E, instead of being admitted to hospital.
